Sage Crossing is a nonprofit dedicated to enhancing the lives of seniors living with dementia through meaningful, service-based programs that foster intergenerational connection, nature-based engagement, and compassionate caregiver support.

We’re excited to launch soon with a group respite program as a first step toward our long-term vision: a vibrant Senior Enrichment Center that empowers individuals with cognitive impairments to age with dignity and purpose, allowing them to experience a sense of belonging at every stage of aging.

As we search for a permanent location, we’re building strong community partnerships and laying the groundwork for a future rooted in holistic care, enriching programs, and strong community ties.

We’re also committed to developing support groups and programs to educate the broader community about brain health and dementia, promoting earlier identification, better understanding, and more inclusive care.

A Holistic Approach to Aging

Sage Crossing is more than a respite program—it’s a Senior Enrichment Center designed to support the full spectrum of aging. Too often, families navigating cognitive change face a fragmented system: medical care in one place, social programs in another, and little coordination between them. This gap leaves older adults under-supported and caregivers overwhelmed.
Through collaborations with medical providers, therapy teams, care management, and home care services, our vision of an integrated campus will serve as a true one-stop destination for individuals and families living with dementia

We envision a future where older adults thrive in an environment rich with connection, purpose, and possibility. Sage Crossing exists to close the gap between healthcare and meaningful daily engagement—creating a new kind of enrichment center where seniors are supported, caregivers are empowered, and every generation finds connection.

Connection is at the core of everything we do. Sage Crossing brings generations together through shared experiences, from nature-based activities to collaborative service projects. Our programming will be enhanced by peer volunteers who offer continuous, compassionate support, expanding our reach and deepening our impact.
This model not only improves care but also helps reduce caregiver burnout and builds a more dementia-inclusive community.

Jill Kozlowski

With nearly 30 years of experience working as a Physical Therapist, Jill Kozlowski has dedicated much of her career to working with the geriatric population. She is also the founder of a non-profit organization, Project Refresh, which provides mobile showers to the unhoused population of Augusta, GA. In her current role as a home health PT, she has received certification through Dementia Capable CARES. Jill is passionate about changing the stigma surrounding dementia and enhancing the quality of care and dignity of those affected. She is also committed to spreading awareness on brain change, providing caregiver support, and creating community education opportunities.

Caitlin Masters

Caitlin Masters is a seasoned Speech Language Pathologist with over 13 years of experience working with adults, driven by a deep commitment to helping those who have lost, or are losing their voice. Her passion for advocacy has expanded into her new role as a Care Manager, where she continues to support individuals and their caregivers within the community. Caitlin is especially focused on preserving dignity and promoting purpose for people living with dementia. She has expanded her knowledge-base by completing training with Sage Care, Positive Approach to Care, and Dementia Capable CARES. Through meaningful connections and community collaboration, she remains dedicated to making a lasting impact in the lives of those she serves.
